1. Review the Neonatal Biorepository Sample and Data Sharing Protocol and Neonatal Biorepository Fees.
2. Complete the REDCap survey to initiate the request: Neonatal Biorepository REDCap Survey
3. Download the appropriate Neonatal Biorepository User Agreement found on Collaborators page, complete, and email to Neonatal-Biorep@uiowa.edu.
4. Neonatal Biorepository personnel will contact you to schedule a meeting to discuss the request further. This initial meeting will be used to help provide information about the availability of samples, phenotypic data, etc. The meeting will also be used to clarify roles and responsibilities, project timeline, and usage fees.
5. You will be asked to present a brief overview of your project to the Neonatal Biorepository Steering Committee. These meetings occur at 3pm CST on the first Monday of the month. If you are unable to attend at the meeting, the committee will review the REDCap survey. Your request may be reviewed by an individual who is not on the steering committee to optimize reviewer expertise and minimize bias. Committee members will vote whether to approve the request. Reviewer and/or committee recommendations may be provided to you after the meeting.
6. After committee approval, Dr. Momany will sign the Usage Agreement and provide a copy to you. An additional meeting may be schedule to finalize details for sample aliquots, shipment, data extraction, etc.
